Based in South East London, our high support forensic facility offers round-the-clock support for male adults. Bridge operates this scheme in partnership with Oxleas NHS Foundation Trust and a dedicated clinical team from the Bracton Centre. To be eligible for a place, clients usually come from one of the South London Boroughs, but we also take referrals from other parts of the UK. They must show a strong commitment to making progress in their lives. Stability and consistency are important factors in promoting recovery, so we give every client a personal key worker. Their role is to help the client achieve the goals agreed during regular CPA (Care Programme Approach) meetings. The key worker not only helps the client while they’re at the facility but continues to support them after they move on.
Our partnership with Oxleas NHS Foundation Trust also provides continuity of support and a solid clinical base for our clients. We have a dedicated clinical team from the Bracton Centre made up of a Consultant Psychiatrist, a social worker, a CPN (Community Psychiatric Nurse), Occupational Therapist and Support Time & Recovery Worker. They meet regularly with our clients and attend weekly multi-disciplinary team meetings to review our clients’ clinical and social activities, carry out risk planning and address any other concerns. Project 106 has a 95% success rate, with the vast majority moving on to live in a more independent environment without reoffending.
